This cartoon uses the double meaning of the word bug. In English if there is a "bug going round", it means there is an illness or infection that a lot of people in one area have at the same time, but a bug is also another word for an insect or other similar small animal. The patient sees a bug spinning and the doctor explains there is a bug going round.
英文說a bug going round，即有一大群人患上了某種病（通常是小病）。Bug又是「蟲子」。醫生玩食字，字面上話有隻蟲在飛來飛去，唔緊要喎。
