A scar is a mark on your skin, which you might have if you hurt yourself or have had an operation, like the man in hospital. When we ask the question "Have a scar?" quickly in English, it sounds like we are saying "Have a cigar?"（offering a cigar）, which is why the man in the bed says he doesn't smoke.
