The girl in this cartoon has a problem as she has eaten a bone. The boy asks if she is choking (to choke is when you cannot breathe because something is blocking your throat). She understands that he has said "Are you joking?" (which sounds similar), and so answers that she is being serious!
女孩子不小心吃下骨頭，卡在喉嚨裡了。男孩子問她是不是噎（choke）到了，她誤以為問她是不是開玩笑（joke），所以回答說：「我很認真！」
